About Us:
Home Carers Alliance provides a range of in-home and community-based support services for people with disabilities who are participants of the National Disability Insurance Scheme (NDIS). These services include Specialised Disability Accommodation (SDA), Supported Independent Living (SIL), Community Access, and Transportation.
About the Role:
As a Service Delivery Coordinator, you will coordinate the delivery of high-quality, person-centred supports for NDIS participants, ensuring services align with their individual needs and goals. Your day-to-day responsibilities will include coordinating participant services and rosters, liaising with participants, families and support workers, maintaining accurate records, and ensuring the smooth delivery of supports that empower participants to live fulfilling and meaningful lives.
About You:
We're looking for someone with excellent communication and organisational skills. Previous experience in disability services or service coordination is highly regarded, along with a genuine passion for improving outcomes for people living with disability.
